List all volunteer community activities in which you have participated during the past four years.

List all school activities in which you have participated during the past four years (e.g. student government, music, sports, etc.) List all other awards received in the past four years.

Submit the information for two people who will write letters of recommendation for you on character, ability and competency. We will reach out and ask them to submit letters of recommendation to us. Recommenders will have until March 25th to submit their letters.

This Application is Confidential and intended for use by the Don Diego Scholarship Foundation staff and interviewing committee members. Your completed application, including supporting documents, MUST be submitted by Midnight on March 15, 2023.
